Cavinton is a drug that improves cerebral circulation, cerebral metabolism and rheological properties of the blood.
It has cerebroprotective effect. It reduces severity of damaging cytotoxic reactions caused by stimulating amino acids; it inhibits functional activity of both cellular transmembrane sodium and calcium channels and NMDA and AMPA receptors. Potentiates neuroprotective effect of adenosine.
Stimulates brain metabolism, increases absorption and assimilation of glucose and oxygen by brain. Stimulates the transport of glucose through the GEB, shifts glucose metabolism in an energetically more favorable aerobic direction. Selectively inhibits Ca2+-calmodulin-dependent cGMP-phosphodiesterase, increases the concentration of cAMP and cGMP in brain tissues, as well as ATP concentration and ATP/AMP ratio. It stimulates cerebral metabolism of noradrenaline and serotonin, stimulates the ascending noradrenergic system and has an antioxidant effect.
The drug selectively increases cerebral blood flow: increases cerebral fraction of minute volume; decreases resistance of cerebral vessels without affecting parameters of total blood circulation (BP, minute volume, HR, PPS).
It does not cause the phenomenon of “stealing”; on the contrary, its use primarily increases the blood supply to the ischemic, but still viable area with low perfusion – the phenomenon of “reverse stealing”.
It improves the tolerance of hypoxia by brain cells by promoting the transport of oxygen to the tissues by reducing the affinity of red blood cells for it.
The drug improves microcirculation in the brain, inhibits platelet aggregation, reduces abnormally high blood viscosity, increases the deformability of red blood cells and blocks their absorption of adenosine.
Pharmacokinetics
Therapeutic plasma concentration is 10-20 ng/ml. When parenteral administration, Vd is 5.3 l/kg. T1/2 – 4.74-5 hours. Easily penetrates through histohematic barriers (including the GEB). It is excreted by the kidneys and through the gastrointestinal tract in the ratio of 3:2.
Indications
In neurology
In ophthalmology: vascular eye diseases (atherosclerosis, angiospasm of the choroid and retina, degenerative diseases of the choroid, retina or yellow spot, arterial and venous thrombosis or embolism, secondary glaucoma).

Active ingredient
Composition
1 ml of concentrate for preparation of solution for infusion contains:
Vinpocetine 5 mg.
Auxiliary substances:
ascorbic acid,
sodium disulfite,
tartaric acid,
benzyl alcohol,
sorbitol,
d/water.
How to take, the dosage
Intravenously, dropwise, slowly (maximum infusion rate 80 drops/min).
It is prohibited to administer intramuscularly and without dilution intravenously.
Physiological solution or solutions containing dextrose (Ringer, Salsol, Rindex, Reomacrodex) may be used to prepare the infusion.
The usual initial daily dose is 20 mg (2 ampoules) in 500 ml of infusion solution. Depending on tolerance, within 2-3 days the dose may be increased to no more than 1 mg/kg/day. The average duration of treatment is 10-14 days.
The average daily dose in case of body weight 70 kg is 50 mg (5 ampoules in 500 ml of infusion solution).
Dose adjustment is not required in case of liver and kidney diseases.
At the end of the course of intravenous therapy it is recommended to continue treatment with tablets Kavinton forte (1 tablet 3 times a day) or Kavinton (2 tablets 3 times a day).
The infusion solution with Cavinton should be used in the first 3 hours after preparation.
Interaction
It is not observed when concomitant use with pindolol, clopamide, glibenclamide, digoxin, acenocoumarol and hydrochlorothiazide; imipramine.
In rare cases, concomitant use with α-methyldopa is accompanied by some increase in the hypotensive effect, and regular BP control is required with this combination.
While there is no data to support the possibility of interactions, caution is recommended when concomitant administration with drugs of central, antiarrhythmic and anticoagulant action.
Cavinton concentrate for infusion solution and heparin are chemically incompatible, so they should not be administered in the same infusion mixture, but anticoagulants and vinpocetine can be treated simultaneously.
Cavinton concentrate for preparation of solution for infusion is incompatible with infusion solutions containing amino acids, so they must not be used to dilute the drug.
Special Instructions
Cavinton infusion solution concentrate contains sorbitol (160 mg/2 ml), therefore blood sugar levels should be periodically monitored in the presence of diabetes mellitus.
In case of fructose intolerance or fructose 1,6-diphosphatase deficiency, use of vinpocetine as concentrate for preparation of infusion solution should be avoided.
The effect of the drug on driving ability. There are no data concerning the effect of vinpocetine on the ability to drive vehicles and operate operating mechanisms.
Contraindications
Side effects
Cardiovascular system disorders: aggravation of existing arrhythmias is possible.
Digestive system disorders: nausea, heartburn, dry mouth.
CNS disorders: headache, dizziness, insomnia, weakness.
Allergic reactions: skin rash, urticaria.
Skin reactions: skin hyperemia.
Overdose
There are currently limited data on vinpocetine overdose.
Treatment: gastric lavage, administration of activated charcoal, symptomatic therapy.
Pregnancy use
It is contraindicated in pregnancy. Vinpocetine penetrates the placental barrier.
In this case its concentration in the placenta and fetal blood is lower than in the blood of the pregnant woman.
In high doses placental bleeding and spontaneous abortions are possible, probably as a result of increased placental blood supply.
In one hour, 0.25% of the dose of the drug passed into the breast milk.
Breastfeeding should be stopped when using the drug.
