Pharmacodynamics
Isosorbide dinitrate causes relaxation of the vascular smooth muscle wall, thus inducing vasodilation. Isosorbide dinitrate acts as a peripheral vasodilator, affecting both arteries and veins. These effects contribute to the deposition of venous blood and decrease venous return to the heart, reducing end-diagnosis and left ventricular volume (reduce preload).
Long-acting Cardiket is characterized by a combination of rapid onset of effect and duration of action.
The benefits of the drug are determined by the fact that the active substance contained in the Cardiket® rolonged-action tablet is rapidly released into the bloodstream. As a result, the drug provides a rapid onset of effect in the treatment of angina pectoris and retains its protective effect for a long time.
Pharmacokinetics
Isosorbide dinitrate is rapidly absorbed when taken orally. Bioavailability when taken orally is 22% (effect of “first passage” through the liver). Onset of action is in 15-30 minutes, Cmax of the substance in blood plasma is reached in 15 minutes to 1-2 hours. As a result of passage through the liver, isosorbide dinitrate is metabolized to form isosorbide-2-monononitrate and isosorbide-5-monononitrate, which have T1/21.5-2 and 4-6 h, respectively. Both metabolites are pharmacologically active. Excreted by the kidneys (almost completely as metabolites). Binding to plasma proteins is 30%.

How to take, the dosage
The dose is chosen individually, as it depends on the stage of the disease and the individual need for nitrates. The tablets are prescribed on 1 tablet.
The drug Cardiket should be taken after meal, without chewing and with small amount of liquid.
If more than 1 tablet/day is prescribed, the interval between doses should be at least 8 hours (to ensure full effect of the drug).
Interaction
Concomitant use of Cardicet Retard with antihypertensive drugs, cyclic antidepressants, MAO inhibitors may increase its hypotensive effect.
In concomitant use Cardicet Retard potentiates the effects of dihydroergotamine and ethanol.
Special Instructions
Cardicet Retard should not be prescribed for the purpose of controlling an attack of angina pectoris.
The severe headache that occurs at the beginning of therapy is in most cases significantly reduced or completely resolved when the drug is continued.
Alcohol should be avoided during treatment with Cardiket.
Impact on the ability to drive and operate machinery
The question about the possibility of engaging in potentially hazardous activities should be decided only after assessing the patient’s individual reaction to the drug.
Contraindications
Side effects
Cardiovascular system disorders: at the beginning of treatment – severe headache (due to dilatation of the cerebral venous system); possibly – tachycardia, orthostatic hypotension.
CNS disorders: somnolence.
Gastrointestinal disorders: in some cases – nausea, vomiting.
Dermatological reactions: in some cases – erythematous rashes.
Overdose
Symptoms: headache, palpitations, dizziness, arterial hypotension, nausea, vomiting.
Treatment: symptomatic.
