Canisone plus is a combined drug with anti-inflammatory, anti-allergic, antibacterial and antifungal (fungicidal) effects.
Gentamicin is a broad-spectrum antibiotic from the group of aminoglycosides, it acts bactericidally. Gram-negative microorganisms – Proteus spp. (indole-positive and indole-negative), Escherichia coli, Klebsiella spp. Salmonella spp., Shigella spp., Campylobacter spp. Staphylococcus spp. (methicillin-resistant).
Sensitive: Enterococcus faecalis, Serratia spp., Pseudomonas spp., Acinetobacter spp., Citrobacter spp.
Resistant: Neisseria meningitidis, Treponema pallidum, anaerobic microbes: Streptococcus spp. (except Streptococcus pneumoniae), Enterococcus faecalis; Providencia rettgeri.
Betamethasone is a glucocorticosteroid, has a local anti-inflammatory, vasoconstrictor, anti-allergic effect.
Clotrimazole is an antifungal agent from the group of imidazole derivatives for topical use. It acts by disrupting the synthesis of ergosterol, which is a part of the cell membrane of fungi. It has a wide spectrum of action. It is effective against dermatophytes, mold fungi, fungi of the genus Candida, Malassezia furfur, Corynebacterium minutissimum, Streptococcus spp, Staphylococcus spp, Trichomonas vaginalis.

How to take, the dosage
Canisone Plus cream should be applied in a thin layer over the entire affected skin surface and the surrounding tissue twice a day – in the morning and at night. To be effective, the Canizon Plus cream should be applied regularly.
The duration of therapy depends on the size and location of the lesion and on the patient’s reaction.
If clinical improvement is not seen after 3 to 4 weeks of treatment, the diagnosis may need to be clarified.
Interaction
The simultaneous use of Canisone Plus with antifungal drugs from other groups decreases the effectiveness of these drugs used topically.
The use of gentamicin can lead to excessively rapid growth of non-susceptible (highly resistant) microorganisms, including fungi.
If superinfection occurs as part of a therapeutic course with gentamicin, the drug should be discontinued and appropriate therapy should be administered.
Special Instructions
Canisone Plus cream is not intended for use in ophthalmology. It is necessary to avoid getting the cream in the eyes.
Long-term topical use of antibiotics can sometimes lead to the growth of resistant microflora, In this case, and if irritation, sensitization or superinfection develops during treatment with Canizon Plus cream, treatment should be stopped and appropriate therapy should be administered. Cross-allergic reactions with aminoglycosidic antibiotics have been observed.
Any side effects common with systemic glucocorticosteroids, including inhibition of adrenal function, may also be seen with topical glucocorticosteroid administration, particularly in children.
The systemic absorption of glucocorticosteroids or gentamicin when applied topically will be higher if treatment is carried out over large areas of skin or if occlusive dressings are used, especially with prolonged treatment or when skin integrity is compromised. It is necessary to avoid applying gentamicin to open wounds and damaged skin. Otherwise, side effects typical for gentamicin during its systemic use may occur. Appropriate precautions should be taken in such cases, especially when treating children. With long-term use of the drug, its withdrawal is recommended to be carried out gradually.
Contraindications
With caution: pregnancy (1st trimester), long-term treatment, use over large areas of skin or when the skin integrity is compromised, use of occlusive dressings (especially in children).
Side effects
Very rarely, burning sensation, erythema, exudation, pigmentation disorders and itching have been reported with Canisone Plus Cream.
Some adverse reactions with topical glucocorticosteroids (especially with occlusive dressings): burning sensation, itching, dry skin, folliculitis, hypertrichosis, acne, hypopigmentation, perioral dermatitis, allergic contact dermatitis, skin maceration, development of secondary infection, skin atrophy, stretch marks, sweat.
Some adverse reactions caused by clotrimazole: erythema, tingling sensation, blistering, peeling, localized swelling, itching, urticaria, skin irritation.
Gentamicin-related adverse reactions: transient skin irritation (erythema, itching), usually not requiring discontinuation of treatment.
Overdose
Symptoms: prolonged use of topical glucocorticosteroids at high doses may suppress adrenal function with development of secondary adrenal insufficiency and symptoms of hypercorticism, including Cushing’s syndrome.
An overdose of clotrimazole when used topically does not lead to any symptoms.
A single overdose of gentamicin is also not expected to cause any symptoms.
Long-term treatment with high doses of gentamicin may lead to growth of insensitive flora.
Treatment: symptomatic. Acute symptoms of hypercorticism are usually reversible. If necessary, correction of electrolyte imbalance is carried out. In cases of chronic glucocorticosteroid toxicity, gradual withdrawal of glucocorticosteroids is recommended.
Pregnancy use
The use of the drug is not recommended in the first trimester of pregnancy. In clinical and experimental studies it has not been found that the use of clotrimazole during pregnancy has adverse effects on the health of the woman and the fetus.
However, the feasibility of prescribing the drug in the second and third trimesters of pregnancy should be decided on an individual basis after consultation with your doctor. The possibility of excretion of clotrimazole with maternal milk has not been studied. Therefore, Kanizon Plus cream for external use should be used with caution in breastfeeding mothers.
In children, the drug should only be used for strict indications and under medical supervision, as there is a possibility of systemic side effects associated with betamethasone.
The use of Canisone Plus Cream, as well as many other glucocorticosteroid-based topical medications, on large surfaces and/or with an occlusive dressing may suppress hypothalamic-pituitary-adrenal system function and may decrease growth hormone excretion as well as increase intracranial pressure.
