Biseptol is a combined antibacterial drug with a broad spectrum of bactericidal action, whose mechanism is due to the blocking of folate biosynthesis in microbial cells.
Sulfamethoxazole disrupts the synthesis of dihydrofolic acid, and trimethoprim prevents the conversion of dihydrofolic acid to tetrahydrofolic acid.
Active against almost all groups of microorganisms – Gram-negative bacteria: Salmonella spp, Shigella spp., Neisseria spp., Proteus vulgaris, Vibrio cholerae, Yersinia spp., Escherichia coli, Corynebacterium spp.; Gram-positive bacteria: Staphylococcus spp. and others.
The drug is also sensitive Chlamydia spp., Actinomyces spp., Klebsiella spp.
Mycobacteria, viruses, most anaerobic bacteria are resistant to the drug.
Indications
Treatment of infectious and inflammatory diseases caused by microorganisms sensitive to the drug:
– Respiratory infections (including. Bronchitis, pneumonia, pulmonary abscess, empyema of pleura);
– otitis, sinusitis;
– meningitis, cerebral abscess;
– infections of urogenital system (including pyelonephritis, infections of the urinary tract).Immune system infections (including pyelonephritis, urethritis, salpingitis, prostatitis);
– Gonorrhea;
– Gastrointestinal infections (including typhoid, paratyphoid, bacterial dysentery, cholera, diarrhea).

How to take, the dosage
The drug is administered at a dose of 6 mg of trimethoprim and 30 mg of sulfamethoxazole per 1 kg of body weight/day.
Children from 3 to 6 months – 2.5 ml every 12 h
Children from 7 months to 3 years – 2.5-5 ml every 12 h
children from 4 to 6 years – 5-10 ml every 12 h
children from 7 to 12 years – 10 ml every 12 h
adults and children over 12 years – 20 ml every 12 h.
The duration of therapy is 10-14 days (in shigellosis – 5 days).
In case of infections caused by Pneumocystis carinii the dose is 120 mg/kg of body weight per day; the drug is taken every 6 hours for 14-21 days.
When the drug is prescribed to patients with impaired renal function (CK of 15-30 ml/min) the dose should be reduced by half.
Interaction
In concomitant use trimethoprim increases the effect of hypoglycemic agents – sulfonylurea derivatives.
When co-administering Biseptol with thiazide diuretics the risk of thrombocytopenia and increased bleeding increases.
Co-trimoxazole increases the anticoagulant effect of warfarin and anticonvulsant activity of phenytoin.
When used together, rifampicin decreases the half-life of trimethoprim.
The co-administration of Biseptol and cyclosporine after renal transplantation worsens the patient’s condition.
Special Instructions
Caution should be exercised when prescribing Biseptol to patients with hepatic and/or renal dysfunction, with allergic reactions in the history, with folic acid deficiency, elderly patients.
The appearance of skin rash or severe diarrhea is an indication for withdrawal of the drug.
Biseptol is not indicated for the treatment of angina caused by streptococcus.
During treatment with Biseptol, adequate fluid intake should be ensured due to the risk of crystalluria and urolithiasis.
When determining creatinine levels using alkaline Jaffe pycrinate while taking Biseptol, note that the result may be 10% higher than the true result.
Control of laboratory parameters
The peripheral blood count should be monitored with prolonged use of the drug.
Contraindications
– severe liver function disorders;
– severe renal function disorders (CK less than 15 ml/min);
– disorders of the hematopoietic system;
– severe diseases of the cardiovascular system. – Severe diseases of the cardiovascular system;
– deficiency of glucose-6-phosphate dehydrogenase;
– pregnancy;
– children under 3 months;
– hypersensitivity to the preparation components.
Side effects
Digestive system: rarely – nausea, vomiting, diarrhea; in single cases – pseudomembranous colitis, cholestatic hepatitis.
Allergic reactions: in single cases – Stevens-Johnson syndrome, Lyell syndrome.
Hematopoietic system: in single cases – reversible thrombocytopenia, leukopenia, neutropenia, agranulocytosis, megaloblastic anemia.
Urinary system disorders: rarely – hematuria, nephritis.
CNS disorders: in single cases – headache, depression, dizziness.
The side effects are usually mild and reversible after discontinuation of the drug.
Overdose
Symptoms: nausea, vomiting, confusion.
Treatment: drug withdrawal, gastric lavage (not later than 2 hours after taking the drug), copious drinking, forced diuresis, taking calcium folinate (5-10 mg/day).
