A combined antibacterial drug of the group of prolonged-acting penicillins destructible by penicillinase. The mechanism of action is based on disruption of synthesis of peptidoglycan – mucopeptide of cell membrane, which leads to inhibition of synthesis of the cell wall of the microorganism, suppression of growth and reproduction of bacteria.

Active against:

• Gram-positive bacteria: Staphylococcus spp. (not producing penicillinase). Streptococcus spp. (including Streptococcus pneumoniae). Corynebacterium diphtheriae. Bacillus anthracis.
• Growth-negative bacteria: Neisseria gonorrhoeae. Neisseria meningitidis. Actinomyces israelii.
• Treponema spp.
• Anaerobic spore-forming bacilli.
• Leishmania.

Mixture of benzathine benzylpenicillin,

benzylpenicillin sodium (or potassium) salt and benzylpenicillin novocaine salt 1200 thousand IU.

How to take, the dosage

Bicillin-3 is injected deep into the upper outer quadrant of the buttock in a dose of no more than 1.2 million units. If two injections are necessary, they are given in different buttocks. Repeated injections are carried out 4 days after the previous injection.

The drug is injected in a dose of 600,000 units once every 6 days.

In the treatment of primary and secondary syphilis

The single dose of the drug is 1.8 million units. The course of treatment is 7 injections. The first injection is given in a dose of 300,000 units; the second injection is given 1 day later in a full dose (1.8 million units). Subsequent injections are given 2 times a week.

In the treatment of secondary relapsed and latent early syphilis

A dose of 300,000 IU is used for the first injection and 1.8 million IU for subsequent injections. The injections are given twice a week. The course of treatment is 14 injections.

Injection of the drug into the body is prohibited.

Regulations for preparation and administration of the solution

Interaction

In concomitant use with bactericidal antibiotics (including cephalosporins, cycloserine, vancomycin, rifampicin, aminoglycosides) synergism is noted; with bacteriostatic antibiotics (including macrolides, chloramphenicol, lincosamides, tetracyclines) – antagonism.

Bicillin-3 reduces the effectiveness of oral contraceptives and ethinyl estradiol (risk of breakthrough bleeding).

The simultaneous use of Bicillin-3 with allopurinol increases the risk of allergic reactions.

Pharmacokinetic interaction: when used concomitantly, diuretics, allopurinol, phenylbutazone, NSAIDs decrease tubular secretion and increase the concentration of penicillin.

Special Instructions

In case of allergic reactions, the treatment should be stopped immediately.

In case of anaphylactic shock antishock therapy is indicated (norepinephrine, GCS, and ventilatory ventilation).

Intravenous and endolumbar administration is not allowed because of the possible development of Wanier syndrome (depression, anxiety, paresthesias, visual disturbances).

In the treatment of venereal diseases, if syphilis is suspected, microscopic and serological studies should be performed before the start of therapy and then for 4 months.

In connection with the possible development of fungal lesions, it is advisable to prescribe antifungal drugs (nystatin, levorin), as well as ascorbic acid and B-group vitamins.

We should be aware that use of the drug in insufficient doses or early termination of treatment leads to the emergence of resistant strains of the pathogen.

Side effects

Allergic reactions: urticaria, angioneurotic edema, erythema multiforme, anaphylactic shock, arthralgia, fever, exfoliative edema.

Digestive system disorders: stomatitis, glossitis.

coagulable system disorders: anemia, thrombocytopenia, leukopenia, hypocoagulation.

Others: with prolonged use – superinfection, mycoses.

Overdose

There are currently no reported cases of overdose of Bicillin-3.

Pregnancy use

There are no data on the use of Bicillin-3 during pregnancy and lactation (breast-feeding).
