Pharmaceutical action:
The drug for external use. Belosalik® lotion has anti-inflammatory, antipruritic, antiedematous, anti-allergic, antiproliferative, immunosuppressive, keratolytic, antimicrobial, antifungal effects.
Betamethasone dipropionate is a synthetic GKS that has anti-inflammatory, anti-allergic, antipruritic and vasoconstrictive effects.
When applied to the skin surface it narrows vessels, reduces itching, decreases release of inflammatory mediators (from eosinophils and mast cells), interleukins 1 and 2 and gamma-interferon (from lymphocytes and macrophages), inhibits activity of hyaluronidase and decreases vascular wall permeability.
It interacts with specific receptors in cell cytoplasm, stimulating synthesis of mRNA, inducing formation of proteins, including lipocortin, mediating cellular effects. Lipocortin inhibits phospholipase A2, blocks the release of arachidonic acid and biosynthesis of endoperoxides, prostaglandins, leukotrienes (contributing to inflammation, allergy and other pathological processes).
Salicylic acid due to its keratolytic action cleanses lesion areas of scales, promotes penetration of betamethasone into the skin and maintains the skin acidic environment, preventing the development of bacterial and fungal infection.

Subacute and chronic dermatoses for which therapy with topical GCS is effective and accompanied by hyperkeratosis and peeling, including:
The drug in the form of a solution for external use is indicated for use on the scalp and in children 6 months and older.
Active ingredient
Composition
1 g of the ointment contains:
Active substances:
Betamethasone dipropionate – 640 µg, which corresponds to the content of betamethasone – 500 µg;
Salicylic acid 30 mg.
Auxiliary substances:
mineral oil,
Vaseline.
How to take, the dosage
The drug is intended for external use.
A small amount of the ointment is spread evenly over the surface of the skin, rubbing slightly. The frequency of use is 2 times a day; in mild cases a single use is sufficient.
The duration of the continuous course of treatment is usually limited to 3-4 weeks. If a longer course of therapy is necessary, it is recommended that the drug be used less frequently, for example, every other day. In the case of chronic conditions, treatment should continue for some time after all symptoms have disappeared, under medical supervision to avoid a recurrence of the disease.
It is possible to repeat courses of therapy during the year.
Interaction
No clinically significant interactions with other medications have been noted.
The simultaneous use of cosmetic and dermatological products for acne therapy, products containing ethanol, or medical soaps with a strong drying effect may in some cases cause skin irritation.
Special Instructions
The ointment is for external use on the skin and/or scalp only.
Contact of the ointment to the eyes and mucous membranes should be avoided.
Long-term use of the ointment on the face is not recommended because of the possible development of rosacea-type dermatitis, perioral dermatitis, skin atrophy and acne.
The use of the drug in the anogenital area should be avoided.
The use of the drug under an occlusive dressing is not recommended except when necessary. If fungal or bacterial microflora develops on the skin, additional use of an antibacterial or antifungal agent is necessary.
Pediatric use
The use of Belosalik in children should be as short as possible with strict precautions, since in children there is a risk of systemic absorption of a proportionally greater amount of the active ingredients due to the predominance of skin area over body weight and insufficient maturity of the epidermis. The drug should not be used in children under bandages and especially under plastinated diapers, which may increase the risk of adverse events.
Influence on driving and operating ability
There are no data on adverse effects of Belosalik on driving and operating ability.
Contraindications
Side effects
The side effects are usually mild.
Dermatological reactions: hypersensitivity reactions may develop (itching, burning, hyperemia); rarely – acne-like changes, hypopigmentation, stretch marks, skin atrophy, hypertrichosis, telangiectasia, secondary skin infections.
In long-term continuous use and in large areas of the skin: Betamethasone may cause systemic adverse reactions associated with suppression of adrenal cortical function.
The drug should be stopped if hypersensitivity or adverse reactions develop.
Overdose
Symptoms: In prolonged continuous use of the combination of betamethasone and salicylic acid, especially in children, on extensive skin surfaces, when applied to skin with compromised integrity or when used under an occlusive dressing, increased absorption of active components and manifestation of systemic effects (secondary adrenal insufficiency, hypercorticism, Icenko-Cushing syndrome) are not excluded.
treatment: cancellation of the drug and symptomatic therapy is recommended.
Pregnancy use
The use of the drug Belosalik (in the form of ointment and/or solution for external use) in pregnant women is allowed in cases when the expected benefits to the mother exceed the risk to the fetus.
In such cases, the use of the drug should be short and limited to small areas of the skin.
In the period of breastfeeding, the use of Belosalik is possible under strict indications, but the drug must not be applied to the skin of the mammary gland before breastfeeding.
