Pharmacological properties

Artoxan is an effective nonsteroidal anti-inflammatory drug (NSAID) with potent anti-inflammatory, analgesic and less pronounced antipyretic effects. The properties of the drug are due to inhibition of both isoforms of cyclooxygenase enzyme, which leads to disruption of arachidonic acid metabolism and blockade of prostaglandin synthesis.

The anti-inflammatory effect is due to reduction of capillary permeability (limits exudation), stabilization of lysosomal membranes (prevents release of lysosomal enzymes that cause tissue damage), inhibition of synthesis or inactivation of inflammatory mediators (prostaglandins, histamine, bradykinin, lymphokines, comlement factors). Reduces the amount of free radicals in the inflammation focus, inhibits chemotaxis and phagocytosis. Inhibits proliferative phase of inflammation, decreases post-inflammatory sclerosing of tissues; has chondroprotective effect.

The drug reduces pain sensitivity in the center of inflammation and acts on the thalamic pain centers, has a desensitizing effect (with long-term use). In rheumatic diseases it relieves pain in the joints at rest and on movement, reduces morning stiffness and joint swelling, improves joint functions and increases joint range of motion.

Pharmacokinetics

It has a long half-life of 60-75 h. It easily passes through histohematic barriers and penetrates well into synovial fluid. It is characterized by high bioavailability – 100%.

Indications

Inflammatory and degenerative diseases of the musculoskeletal system accompanied by pain syndrome:

– rheumatoid arthritis, gouty arthritis, ankylosing spondylitis (Bechterew’s disease);

– infectious nonspecific polyarthritis;

– osteoarthritis, osteochondrosis;

– tendinitis, bursitis, myositis, periarthritis;

– arthralgia, neuralgia, myalgia, ischalgia, lumbago;

– injuries, burns.

Lyophilized powder for preparation of solution for injection, complete with solvent.

How to take, the dosage

It is indicated for intramuscular or intravenous administration.

Prescribed in a dose of 20 mg once daily, maintenance dose: 10 mg per day.

In case of severe pain syndrome, the dose can be increased to 40 mg once daily.

In acute attacks of gouty arthritis: 20 mg 2 times daily for the first 2-3 days, then 20 mg once daily for 5 days.

Interaction

The drug enhances the effect of lithium drugs, indirect anticoagulants (prothrombin index should be controlled).

When using with diuretics, the possibility of sodium and water retention in the body should be taken into account.

Special Instructions

An extended treatment with Artoxan requires monitoring of liver and kidney function. Bleeding time may increase, which should be taken into account during surgical interventions.

Particular caution should be exercised when prescribing the drug to persons under 18 years of age due to the lack of sufficient information on their use of tenoxicam.

Impact on the ability to drive a motor vehicle and other machinery During treatment with the drug, caution should be exercised when driving a motor vehicle and engaging in other potentially dangerous activities that require increased concentration and rapid psychomotor reaction.

Contraindications

– hypersensitivity to tenoxicam or other NSAIDs;

– gastrointestinal erosive and ulcerative lesions;

– gastrointestinal bleeding;

– pregnancy;

– lactation period;

– marked liver and kidney function disorders;

– renal failure;

– heart failure;

– blood diseases.

Side effects

Digestive system disorders: abdominal pain and discomfort, nausea, flatulence.

Nervous system disorders: dizziness, headache.

Allergic reactions: itching, urticaria, erythema, Stevens-Johnson and Lyell syndromes. Laboratory characteristics: increased plasma concentrations of creatinine and urea, increased “hepatic” transaminase activity.

Overdose

There have been no cases of overdose.
