Amlovas is antispasmodic, antihypertensive, vasodilator, antianginal.

Blocks entry of calcium ions through cell membranes into the smooth muscle cells of the myocardium and vessels. It has a direct relaxing effect on vascular smooth muscle, dilates peripheral arterioles, resulting in decreased myocardial pressure, reduces the load on the heart and myocardial oxygen demand, dilates coronary arteries, increasing the flow of oxygen to the myocardium.

Pharmacokinetics

After oral administration, it is quickly and almost completely (90%) absorbed from the gastrointestinal tract. Bioavailability is 60-65%, C< sub> max in plasma is reached after 6-12 h, equilibrium concentration – after 7-8 days.

The volume of distribution is about 20 l/kg, binding to plasma proteins is more than 95%, penetrates through the BBB and into breast milk. Metabolized mainly in the liver with the formation of inactive metabolites, T< sub>1/2 – 35-45 hours. It is excreted by the kidneys (10% – unchanged and 60% – as metabolites) and also with the bile and through the intestine.

Indications

Arterial hypertension (monotherapy and in combination with thiazide diuretics, ACE inhibitors or alpha- and beta-blockers), angina pectoris (stable and unstable, including Prinzmetal or variant), dilated (non-ischemic) cardiomyopathy with severe chronic heart failure.

Special instructions

You should refrain from using the drug in pediatric practice, since the safety and effectiveness of its use in children have not been determined.

Caution should be used in patients with impaired liver function, aortic stenosis, and chronic heart failure.

Contraindications

Hypersensitivity, arterial hypotension.

Side Effects

From the nervous system and sensory organs: dizziness, headache, drowsiness, feeling tired; rarely – mood changes, asthenia, convulsions, myalgia, blurred vision, paresthesia.

From the cardiovascular system and blood (hematopoiesis, hemostasis): palpitations, hot flashes.

From the gastrointestinal tract: nausea, abdominal pain, dyspeptic symptoms; rarely – gingival hyperplasia, increased levels of liver transaminases, jaundice.

From the genitourinary system: increased frequency of urination, swelling, impotence.

From the skin: allergic reactions (skin rash, itching), rarely – erythema multiforme.

Other: rarely – shortness of breath, gynecomastia.

Overdose

Symptoms: severe arterial hypotension.

Treatment: maintaining the function of the cardiovascular system, monitoring indicators of heart and lung function, elevated position of the extremities, control of blood volume and diuresis, use of vasoconstrictors, administration of calcium gluconate (iv).
