Dihydropyridine derivative – BCC of II generation, has antianginal and hypotensive effects.

Binding to dihydropyridine receptors, it blocks calcium channels, reduces transmembrane transport of calcium ions into the cell (more so into vascular smooth muscle cells than into cardiomyocytes).

The antianginal action is caused by the dilation of coronary and peripheral arteries and arterioles: in angina pectoris it reduces the severity of myocardial ischemia; by dilation of peripheral arterioles it reduces the PPS, decreases the preload of the heart, reduces myocardial oxygen demand.

Dilating the main coronary arteries and arterioles in unchanged and ischemic areas of the myocardium, increases the flow of oxygen to the myocardium (especially in vasospastic angina); prevents the development of coronary artery constriction (including that caused by smoking).

In patients with angina a single daily dose increases exercise time, slows down the development of angina and “coronary” ST-segment depression, reduces the frequency of angina attacks and nitroglycerin consumption.

It has a long-term dose-dependent hypotensive effect.

The hypotensive effect is due to a direct vasodilatory effect on vascular smooth muscle.

In arterial hypertension, a single dose provides clinically significant BP reduction for 24 hours (in “lying” and “standing” positions).

It does not cause a sharp decrease in BP, decreased exercise tolerance, and decreased left ventricular ejection fraction.

Limits the degree of myocardial hypertrophy of the left ventricle, has anti-atherosclerotic and cardioprotective effect in CHD.

It does not influence myocardial contractility and conduction, does not cause reflex increase of HR, inhibits platelet aggregation, increases glomerular filtration rate, has weak natriuretic action.

In diabetic nephropathy it does not increase the severity of microalbuminuria.

It does not have adverse effects on metabolism and plasma lipids.

The time of onset of effect is 2-4 hours, the duration of effect is 24 hours.

Cardiovascular system disorders: palpitations, shortness of breath, marked decrease in BP, fainting, vasculitis, edema (swelling of ankles and feet), flushes of blood to the face, rarely – rhythm disturbances (bradycardia, ventricular

tachycardia, atrial flutter), chest pain, orthostatic hypotension, very rarely – development or worsening of heart failure, extrasystole, migraine.

CNS disorders:

Headache, dizziness, fatigue, drowsiness, mood changes, seizures, rare – loss of consciousness, hypoesthesia, nervousness, paresthesia, tremor, vertigo, asthenia, malaise, insomnia, depression, unusual dreams, very rare – ataxia, apathy, agitation, amnesia.

Digestive system disorders:

Nausea, vomiting, epigastric pain, rare – increased liver transaminases and jaundice (due to cholestasis), pancreatitis, dry mouth, flatulence, gum hyperplasia, constipation or diarrhea, very rare – gastritis, increased appetite.

To the genitourinary system:

Rarely – pollakiuria, painful urge to urinate, nycturia, impaired sexual function (including decreased potency); very rare – dysuria, polyuria.

Skin disorders:

very rarely – xeroderma, alopecia, dermatitis, purpura, changes in skin color.

Allergic reactions:

cutaneous itching, rash (including erythematous, maculopapular rash, urticaria), angioedema.

Musculoskeletal system:

rarely – arthralgia, arthrosis, myalgia (with long-term use); very rarely – myasthenia.

Others:

Rarely, gynecomastia, uricemia, weight gain/decrease, thrombocytopenia, leukopenia, hyperglycemia, visual disturbances, diplopia, conjunctivitis, eye pain, tinnitus, back pain, dyspnea, nosebleed, increased sweating, thirst;

very rarely – cold clammy sweat, cough, rhinitis, parosmia, impaired sense of taste, impaired accommodation, xerophthalmia.
