Sodium bicarbonate has alkaline properties and increases the alkaline reserve of the blood.

In internal use, it quickly neutralizes hydrochloric acid of gastric juice and has a rapid, but short-lived antacid effect. It irritates the receptors of the gastric mucosa, increases gastrin release with secondary activation of secretion, may cause discomfort in the stomach (due to its distension) and belching.

By reducing the viscosity of sputum due to a shift in the alkaline side of the reaction of bronchial mucus has expectorant action

In absorption, leads to the development of alkalosis. Alkalization of urine prevents deposition of uric acid in the urinary tract.

Indications

Ulcer disease;

High acidity of gastric juice;

How to take, the dosage

It is used orally, parenterally, topically externally. Dosing regimen depends on the indication and method of administration.

Interaction

Concomitant use decreases urinary excretion of amphetamine due to increased urinary pH under the influence of sodium bicarbonate.

In oral administration of sodium bicarbonate with lithium carbonate in prescribed maintenance doses, plasma lithium concentration may decrease due to the effect of sodium ions.

In concomitant use with methotrexate urinary excretion of methotrexate is increased and its toxic effect on the kidneys is reduced due to increase in urine pH under the influence of sodium bicarbonate.

The absorption of tetracyclines is reduced with concomitant ingestion.

Ephedrine excretion may be delayed due to increase in urine pH caused by sodium bicarbonate and increased risk of side effects (tremor, anxiety, sleep disturbance, tachycardia).

When sodium bicarbonate is administered by IV drip, the antihypertensive effect of reserpine may be enhanced.

Special Instructions

It is not recommended to use systematically due to the fact that during neutralization of gastric hydrochloric acid with sodium hydrogen carbonate carbonic acid is released, which has a stimulating effect on the receptors of the gastric mucosa, increases gastrin secretion and may cause a secondary increase in secretion.

In addition, with prolonged regular use, it is possible to alkalize urine and increase the risk of phosphate stone formation.

Intensive excretion of CO2 may provoke perforation of the walls of the gastrointestinal tract.

In patients with concomitant heart or kidney disease, excessive sodium intake causes edema and heart failure.

Contraindications

Side effects

Long-term use may result in alkalosis (sometimes uncompensated) accompanied by loss of appetite, nausea, vomiting, epigastric pain, restlessness, headaches, and in some severe cases tetanic convulsions; increased BP, flatulence (when taken orally).
