Clarithromycin is an antibiotic of macrolide group that inhibits protein synthesis in a microbial cell by interacting with 50S ribosomal subunit of bacteria. It is active against Streptococcus agalactiae (pyogenes, viridans, pneumoniae), Haemophilus influenzae (parainfluenzae), Neisseria gonorrhoeae, Listeria monocytogenes, Legionella pneumophilia, Mycoplasma pneumoniae, Helicobacter pylori, Campilobacter jejuni, Chlamydia pneumoniae (trachomatis), Moraxella (Branhamella) catarrhalis, Bordetella pertussis, Propionibacterium acnes, Mycobacterium avium, Mycobacterium leprae, Staphylococcus aureus, Ureaplasma urealyticum, Toxoplasma gondii.
When administered orally clarithromycin is metabolized to form the main metabolite 14hydroxyclarithromycin which has the same or 1-2 orders of magnitude less (depending on the type of microorganism) antimicrobial activity than the unchanged substance.
Indications
Infections caused by microorganisms sensitive to clarithromycin – upper respiratory tract infections (laryngitis, pharyngitis, tonsillitis and sinusitis);
lower respiratory tract infections (bronchitis, bacterial pneumonia);
skin and soft tissue infections (folliculitis, furunculosis, impetigo, wound infection);
infections caused by Mycobacterium avium, Chlamydia trachomatis, Helicobacter pylori, Ureaplasma urealyticum.

Contraindications
Hypersensitivity to macrolides.
Side Effects
Possible nausea, vomiting, pain in the epigastric region, diarrhea, transient increase in the activity of liver transaminases, skin rash, headache.
Interaction
With the simultaneous administration of clarithromycin with theophylline and carbamazepine, an increase in the content of the latter in the blood plasma was noted.
Overdose
Symptoms: nausea, vomiting, diarrhea.
Treatment: rinse the stomach as quickly as possible, prescribe symptomatic therapy.
