Cardionate has metabolic, cardioprotective, antianginal, antihypoxic, adaptogenic, improves cerebral circulation.

Indications

In complex therapy: coronary heart disease (angina, myocardial infarction), chronic heart failure, cardialgia against myocardial dystrophy.

In complex therapy: cerebrovascular disorders (ischemic stroke, cerebrovascular insufficiency).

In complex therapy: acute circulatory disorders in the retina, hemophthalmos and retinal haemorrhages of various etiology, thrombosis of the central retinal vein and its branches, retinopathies of various etiology (including diabetic and hypertensive) – only for parabulbar injection.

Disabled performance, physical overexertion (including in athletes).

The withdrawal alcohol syndrome (as part of complex specific therapy).

How to take, the dosage

Because of the possible excitatory effect the drug is recommended to be used in the morning.

Stenocardia and myocardial infarction (in combined therapy): intravenously (IV) 0.5-1 g (5 – 10 ml of 100 mg/1 ml solution for injection) once daily, then switch to oral administration.

Cronic heart failure (in combined therapy): by injection 0.5-1 g (5-10 ml of solution for injection) 1 time per day: the course of treatment – 10-14 days, then the drug is taken orally.

Cardialgia against myocardial dystrophy: intravenously 0.5-1 g (5-10 ml of solution for injection) once daily or intramuscularly 0.5 g 1-2 times daily; course of treatment – 10- 14 days, then the drug is taken orally.

Cerebral circulation disorders (as part of combined therapy): acute phase – by injection 500 mg (5 ml of solution for injection) once daily for 10 days, then it is possible to switch to oral administration; chronic cerebral circulation disorders’, by injection 500 mg (5 ml of solution for injection) once daily; the course of treatment – 10-14 days, then it is possible to switch to oral administration.

Vascular ocular pathology and retinal dystrophy (in complex therapy): parabulbarine 0.5 ml of the solution for injection for 10 days.

Physical and mental overload: intravenously 0.5 g (5 ml of solution for injection) once a day; the course of treatment is 10-14 days. If necessary the course is repeated in 2-3 weeks.

Antebriety alcohol syndrome: by 500 mg (5 ml of solution for injection) 2 times a day; the course of treatment is 7-10 days.

Interaction

When used together, meldonium increases the effect of antianginal drugs, some hypotensive drugs, and cardiac glycosides.

In concomitant use of meldonium with nitroglycerin, nifedipine, alpha-adrenoblockers, antihypertensive agents and peripheral vasodilators, moderate tachycardia and arterial hypotension may occur (caution is required with these combinations).

Special Instructions

The drug should be used with caution in patients with liver and/or kidney disease, especially for prolonged periods of time.

Long-term experience in the treatment of acute myocardial infarction and unstable angina in cardiology departments shows that meldonium is not a 1st-line drug for acute coronary syndrome and its use is not acutely necessary.

The effect on the ability to drive and maintain other mechanisms

There is no evidence of adverse effects of meldonium on psychomotor reaction speed.

Features

After intravenous administration maximum drug concentration in blood plasma is achieved immediately after administration.

There are no data on bioavailability of the drug after intramuscular administration.

It is metabolized in body with formation of two main metabolites that are excreted by kidneys.

Half-life period is 3-6 hours.

Contraindications

Hypersensitivity to the drug; increased intracranial pressure (with impaired venous outflow and intracranial tumors); pregnancy, lactation, age under 18 years.

Side effects

Cardiovascular system: rarely – tachycardia, BP changes.

CNS disorders: rarely – psychomotor agitation.

Digestive system disorders: rare – dyspeptic symptoms.

Allergic reactions: rare – skin itching, redness, rash, swelling.

Overdose

There are no known cases of overdose of Cardionate.

The drug is mildly toxic and does not cause side effects that are dangerous to the health of patients.

Pregnancy use

In pregnancy and during lactation (breastfeeding) the use of the drug is contraindicated.
