Sodium chloride – detoxifying.
Pharmacodynamics
It has a detoxifying and rehydrating effect. Replenishes sodium deficiency in various pathological conditions. Sodium chloride 0.9% solution isotonic to human plasma and therefore is quickly eliminated from the vascular bed, only temporarily increasing the BOD (effectiveness in case of blood loss and shock is insufficient).
Pharmacokinetics
The sodium concentration is 142 mmol/L (plasma) and 145 mmol/L (interstitial fluid), the chloride concentration is 101 mmol/L (interstitial fluid). It is excreted by the kidneys.
Indications
Restoration of water-salt balance
- isotonic extracellular dehydration;
- hyponatremia;
- dilution and dissolution of parenterally administered drugs (as a base solution).

How to take, the dosage
Isotonic sodium chloride solution is administered in/in, n/a and in enemas, and also used for washing wounds, eyes, nasal mucosa. Most often administered by ingestion, depending on the clinical situation – up to 3 liters per day.
Hypertensive sodium chloride solution is administered by ingestion. A single dose for IV/injection can be 10-30 ml. In conditions requiring immediate replenishment of sodium and chloride ions, the drug is administered by IV drops in a dose of 100 ml.
Interaction
Compatible with colloidal hemodynamic blood substitutes (mutual enhancement of effect).
When mixing with other medications, visual compatibility must be controlled (however, invisible and therapeutic incompatibility is possible).
Special Instructions
With caution use large amounts of sodium chloride in patients with impaired renal excretory function, with hypokalemia. Administration of large amounts of the solution may lead to acidosis, hyperhydration, increased excretion of potassium from the body.
Hypertensive solution is not used per/v and m/m.
Per prolonged use requires monitoring of plasma electrolyte concentration and daily diuresis.
The temperature of the infusion solution should be 38 ° C.
Contraindications
- hypernatremia, acidosis, hyperchloremia, hypokalemia, extracellular hyperhydration;
- circulatory disorders, threatening brain and pulmonary edema;
- Cerebral edema, pulmonary edema, acute left ventricular failure, concomitant administration of high-dose GCS.
When other drugs are added to the solution, the contraindications for these drugs should be considered.
With caution: decompensated chronic heart failure, arterial hypertension, peripheral edema, preeclampsia, chronic renal failure (oligo-, anuria), aldosteronism and other conditions associated with sodium retention in the body.
Side effects
Acidosis, hyperhydration, hypokalemia.
